The first planet was successfully colonized, but instead of colonizing the second planet, which had the requisite number of colonies, the improved colonize card was instead simply added to the planet as a colony.

Report history
The text ask you to select a card to settle or colonize, but i didn't realize you may select two cards, one to settle other to colonize.
I think it's better to do it in the order described on the card. First, select a planet to settle, and then select a planet to either settle or colonize.
1. Choose a planet to settle, or skip.
2. Choose a planet to settle or add a colony to.
